Registering with the Practice
If you are new to the area and wish to register, please come to reception and ask for a form. One form is needed for each household member. You should bring your NHS medical card with you, and if you are not a UK Citizen, then you will need to bring your passport. You will be asked to complete a new patient health check, so that information of your medical history, along with any medication you are taking, can be taken prior to your medical records arriving.
GMS1(New Patient Registration Form)
Your Data Matters to the NHS, please download this leaflet for more information: Patient Information – opting out
You will require two forms of identification:
- a passport or Full driving Licence (both parts)
- a utility bill with your name and address on it
Named Accountable GP
From 1 April 2014, the practice was required under the terms of the GP contract to ensure all patients aged 75 and over were allocated a Named Accountable GP. From 1 April 2015 this was extended to all registered patients in the practice. Your Named Accountable GP will be the named doctor you are registered with. If you wish to be told the name of your Accountable GP, please ask the receptionists when you are next in the surgery.
Having a named GP does not prevent you seeing any other doctor in the practice and you should continue to book appointments as you normally would. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor. If you do wish to express a preference please let the receptionist know and we will try to accommodate your request. If you do not hear back from the practice we can confirm that this change will be made
We are always happy to see any relatives or friends staying with you if they need a Doctor or Nurse. They will need to fill out a short form for the information to be returned to their own Doctor.
Please arrive 10 minutes before your appointment to allow time for completion of the necessary forms.
